Library Card Application

Note: This form is for new registrations only. If you already have a library card from a library in Rock County, you do not need to fill out this form.

Residents of other municipalities in Rock County must go to their own city's library (City of Beloit, Clinton, Edgerton, Evansville, Janesville, or Orfordville residents). This form is for residents of Milton and all surrounding townships. Please call the library at 608-868-7462, if you have any questions.

To pre-register for a library card, please fill in the information below and click the submit button.

To pick up your library card bring a picture ID, like a driver's license, with your current address to the Milton Public Library. If your current address isn't on your license also bring a piece of official mail that includes your name and current address.

If you are under 16, you need a parent or guardian to sign the form for you. Library cards are issued at no charge.
