BadgerCare Plus Community Outreach - Rock
County, Wisconsin

Kids
need comprehensive preventative health care so they can grow up to be healthy
and strong.
l
Since February 1, 2008, all kids in Wisconsin
who do not have access to health insurance have been able to get the health
care they need at a price families can afford through BadgerCare Plus.
l
This new
program is a consolidation of the Wisconsin Medicaid, BadgerCare, and Healthy
Start programs and is for children under 19 years of age, pregnant women, and
the parents and caretakers of children.
l
Families at higher income levels will
pay premiums and co-payments for certain services.
l
Assistance to complete online
BadgerCare Plus applications will be provided throughout Rock
County
at various community application sites starting June 23rd.

BadgerCare+ Application Sites in Rock County
The BadgerCare+ Outreach Program has established community application sites
throughout Rock County to assist in applying for this free or low-cost health
care program. Assistance will be provided at the following locations. Please note: days and times of application sites being
subject to change.
Janesville:
-Exchange Family Resource Center, 2020 E. Milwaukee St.
9 a.m. until 11 a.m., Mondays *Call 741-3517 for an appointment
-Hispanic Outreach Program, Y.W.C.A. of Rock County; 1735 S. Washington St.
*Appointments only, serving the Hispanic population; call 752-5445 x215 or x 222 between 9 a.m. and 4 p.m. Monday-Friday
-Mercy Clinic South, 849 Kellogg Ave.
5 p.m. until 7 p.m., Thursdays
Beloit:
-Stateline Pregnancy Clinic, 347 W. Grand Ave.
9 a.m. until 3 p.m., Monday, Tuesday, and Wednesdays
-Mercy Beloit Medical Center, 2825 Prairie Ave.
6 p.m. until 8 p.m., every 1st and 3rd Wednesday
Clinton:
-Clinton Public Library, 214 Mill St.

Edgerton:
-Edgerton Hospital and Health Services, 313 Stoughton Rd.
4 p.m. until 6 p.m., Tuesdays

Milton:

2:30 p.m. until 4:30 p.m., every 2nd and 4th Friday
5:30 p.m. until 7:30 p.m., every 1st and 3rd Monday
